During the evaluation, a licensed therapist will evaluate and identify your needs. Areas being evaluated may include any or all of the following: speech, language, voice, fluency, swallowing, feeding, orthopedic, neurologic, or sensory disorders. After the evaluation, the therapist will review findings and whether or not therapy is warranted. The evaluation report will be sent to the primary care provider for review and approval. From that point on, therapy (if warranted) will be set up on a consistent basis.

If you are a current, or former, patient of The Speech Path and need to request a copy of your therapy records, you can do so by downloading the following form. Make sure to fill it out completely and fax it back to us at the number on the bottom of the page. If you have any questions regarding this form, please let us know. All records requests will be handled within 2 business days.
If you are seeking Speech or Occupational Therapy services for your child, talk to your pediatrician regarding concerns. Your pediatrician will need to send The Speech Path a referral. You can ask them to fill out the referral form below. We will contact you after we receive this form to schedule an appointment to complete an evaluation.
